How are languages lost?

In history text books I constantly hear about how the ability to read, write, and communicate all suddenly disappear if the culture goes through a dark age. My question is does this happen instantaneously? Do they just wake up and say "hey do you remember how to write? I don't". Or if it is gradual, why would the stop such a vital way of communication.
