I am considering taking a second course of accutane for acne. Should I? Will it be a permanent cure this time?

I had mild but stubborn acne that was completely cleared for a year and a half after I finished my first course of accutane. Now the acne is returning. It is sparse but the zits are big and painful. I may consider going on accutane again, but only if the second time will cure it completely. Otherwise it isn't worth the money and annoyance. What are your experiences? Please help!
